How is the chainline? Don't eyeball it.

Look to see if there are any signs of cracks, especially in the welds. Or bent chainring.

Check your chainring make sure there are no bent teeth, and also check chain tension. But if you are pedaling when it happens sounds like bent chainring or bent teeth on chainring.

Make sure none of your chainring bolts are missing.

What's the condition of your chain? If it's over worn it tends to fall.
It happened to me on my beloved Surly, using a PC-1 chain I had refused to replace.

I have this chain PC-1 about two months. I need to check it.
    Thank you for your information.

Thank you for your link. I found out my chainline off a lot. It looks like working now.

i have found a 1/8" chain sometimes will drop off a 3/32" ring even if everything is pretty close to perfect otherwise.

i'd recommend buying the chain your front chainring is built for. probably 3/32"
