What can be wrong with my disc brakes or is it the levers?

Well I have a Trek Wahoo Gary Fisher collection 11'. It comes with Promax Hornet Hydraulic brakes. My problem is that after a ride session I am able to pull the lever almost all the way back to the handle bar to be fully engaged. The adjustment screw on the lever seems to loosen it's way during the ride I guess because I adjust it by screwing it in then by the end of the ride it comes back out. This is only on my rear brake. I use the rear brake a lot.

Also I can't seem to find brake pads for the Promax brake setup. Can I use any brake pads? I'm hoping changing the brake pads will eliminate the squeel. I cleaned them, everything seems to be tight and nothing really loose. But, after I clean the rotors and pads it doesn't squeel. How often do you guys clean your brakes?
