You would need to use the adapter above in addition to a 1-1/8" headset to install a 1-1/8" fork on a cannondale with a headshock. Also a good idea to measure axle to crown height on the old headshock to find something that will set him up in a similar riding position

You should still be able to rebuild it if it's not completely trashed as well, in case that's a consideration.

A simpler way would be to keep the current external cups and replace the bearings with the Cannondale KP058 kit. I also offer head angle reducing headsets compatible with those cups: https://www.pinkbike.com/buysell/2118769/
IIRC the headshok head tube, with cups removed, is a 49mm standard (49.6mm inner diameter) There may be 1,1/8" headset for that size.

headset needed for replacing a Cannondale Headshok

Your choice of a reducer headset will depend on the current installed headset. The Cannondale KP058 is a 1.5" SI (System Integrated) reducer to use a 1 1/8" fork. The Cannondale KP205 is a 1.5" SI reducer for a tapered fork. The Cannondale KP119 is another 1.5" SI reducer for for a 1.5" steerer tube. The standard 1.5" headset (with the SI on top) uses a different set of headset reducers, e.g., the HD232 for 1 1/8" steerer tubes, etc. You can find more details at:
