Mine had to be adjusted all of the way in IIRC, but it worked fine (Shimano XT).
I assume yours doesn't have enough adjustment range if you have it all the way in but can still shift up?

Ha. So what was it, adjusting it the wrong way or adjusted the low limit in instead?

Not sure exactly, I was messing with 2 nights ago, not really knowing what it was doing, and couldn't get it to work. Then I watched a few videos last night while drinking beer on how to properly adjust a front derailleur, you know starting from scratch, loosen the cable, set the low limit blah blah blah. After getting that set and it shifting smooth from little ring to middle ring, I had a few more and forgot to set the high limit. After I posted today I took another look, and sure as sh't I hadn't set the high limit. A few turns of the screw and viola! No more shifting onto my bash guard!
If you can't laugh at yourself, who can you laugh at right?
