    derailer issue?

when I am pedaling hard uphill or putting a lot of force on the crank, my chain keeps trying to jump, or maybe trying to shift gears on its own. not really sure what this issue is, i am fairly new to biking. I bought a 2012 diamondback overdrive last november.

is this just the derailer needing tuning? the bike is fairly new so i doubt it is a bent chain, and all the teeth look okay. any advice on how to fix it?

    here is what I have:

    #1 bent Derailleur hanger
    #2 worn chain-cassette
    #3 cable worn/thread broken

if it just started happening and the bike is fairly new it's almost 90% the hanger got tweeked on a stick or rock or just got bumped moving the bike around...
Going to try and bring Trail Tire TV back. go take a look... http://trailtiretv.blogspot.com/
