What am I missing (bleed question)

Bought an Avid bleed kit for my XO brakes. Read instructions and even took the wifes Ipad out to the garage so I can follow the video while I'm working on them. Everything went fine and the brake lever returned to its old self right up until the time I take out the caliper spacer and put the pads and wheel back on then the lever pulls all the way to the bar and no pad contact at all to the rotor. 2 more attempts ended with the same results.

Finally did the bleed without the caliper spacer and left the pads and wheel in place and everything works great now. The spacer I used is the only one that will fit the caliper. What am I missing or not doing to cause the bleed not to work while using the spacer?
It's such a fine line between idiocy and genius.

You need to pull the brake lever several times after putting the wheel back on. The pistons move out a tad with each pull. The lever should firm up after the pads move out to the rotor.
