Loosing a brake pad while riding?

A friend of mine told me that he lost a brake pad while riding???
I was skeptical until yesterday while going downhill pretty fast he lost it again. We found the brake pad and put it back and had no problem after that.
I made sure that the spreader clips was really pushing the pads away from each other.
Nothing was really obvious except that the brake pad had wear only on the first quarter of its surface???

???? I have had the pad come off the backing plate....but I have never had the backing plate and the pad come out of the caliper.

Happened to me with Hayes Stroker Trail brakes. I squeezed the lever and the pad was gone. The rotor tore the pin that holds the pad to the piston off
    No moss...
