I noticed the same thing on my Guardian yesterday. Where the RD mounts to the hanger is loose. It's a 5mm allen bolt I think, tighten it up. Mine was loose, causing bad shifts
well...I guess something wrong with the thread on hanger.
bolt tightening go well for 2/3 but no good for last couple of turns.
it looks like stripped or mis screwed so RD hanging tilted.
I don't know what happened but...I guess it tightened wrong.
Anyway I placed an order for extra hanger
I can't tell completely from the photo, but it looks like it is aligned vertically OK but that it is off in the horizontal plane due to the RD hanger threads being stripped (?).
If that's the case a new RD hanger should definitely help.

I've had the cir-clip that holds the b-adjustment stop come loose more than one SRAM derailleur. I can't tell for sure from the picture, but it certainly looks like that's a possibility. It's fairly easy to fix but be careful removing or install the cir-clip. They like to fly across the garage and hide.
